ABFM Performance Improvement Activity: Maximizing Adult Vaccinations with the 4 Pillars Immunization Toolkit

This module will fit requirements for ABFM Performance Improvement Activity credit and is designed for physicians who wish to maximize adult vaccination rates.

There are 4 phases to this module:

  1. Review of vaccination barriers and determination of your practice’s current vaccination rates.
  2. Select the evidence-based strategies you would like to implement.
  3. Implement the strategies chosen.
  4. Evaluation of improvement.

 

The Toolkit’s evidence-based 4 Pillars incorporated in this module are:

  1. Convenient vaccination services
  2. Messaging to patients
  3. Enhanced office systems
  4. Motivation

This Toolkit was developed with up-to-date CDC guidelines and has proven effective.

Requirements: 
  • Read and reflect upon 1 case study.
  • Test your knowledge regarding vaccination systems & science.
  • Conduct a minimum of 10 chart audits.
  • Read a self-learning module.
  • Implement selected intervention strategies for at least two weeks.
  • Conduct a post-audit.
  • Complete a short evaluation survey.

Payment Information:

We do not charge a fee for you to take this module. Payments will be handled by the ABFM through their website. More information regarding payment for this module will be provided upon enrollment.

This activity has been approved by the American Board of Family Medicine for Maintenance of Certification for Family Physicians Part IV credit. Term of approval is for two years beginning July 20, 2015, with the option for yearly renewal thereafter.
