In order to view Your Practice's 4 Pillars™ Job Site & Plan homepage, you must be a registered user of this site. Registering is free and easy; accomplished in three short steps.
Step 1. Click on the "Register" button located in the top right-hand corner of the homepage. This button is circled and shown below in red.
Step 2. Next, simply fill in your information in the fields noted below. Required fields are noted with an asterisk however feel free to also complete "Your Practice Job." If you are the Immunization Champion, Office Manager, Head Nurse or Lead Physician, please check your corresponding role under the field "Project Management Role."
Step 3. After completing all the fields, click on the button titled "Create new account," which is located at the bottom of the page.
Congratulations! You have now created your user account on the 4 Pillars™ Immunization website. Anytime you want to view your practice's progress in increasing your patient immunization rates simply login and navigate to Your Practice's 4 Pillars™ Job Site & Plan homepage.
